Fixed: profile photo in the nav

A little while ago we added your profile photo to the account menu in the top nav. For some of you it never actually showed up, no matter how many times you reloaded the page or checked your account settings. The problem was that your photo was only being picked up at the moment you logged in. If you'd already been logged in before we added photos, or before an earlier attempt to fix this, your session just never got the memo. That's fixed now: the nav always shows your current profile photo, checked fresh every time, so this can't happen again. If you still see your initials instead of your photo, log out and back in once and it'll pick up right away.
